Gas for brake: In highly tense situations right before hitting a wall, pressing the wrong button and accelerating into it instead, instantly failing a situation that was easily salvageable.

Forgetting to straighten the wheel: After reversing and getting the angle right, hitting the gas without straightening the steering wheel first, causing the car to veer off again and scrape a neighboring car.

Stubbornly forcing the wrong route: Some spots look like you can drive in head-first, but the designer intended for you to reverse in. If driving in head-first fails five times, change your approach and try reversing.
